Incoming Students

The following steps must be completed prior to attending classes:

1. Download the Immunization History Form (required)

  • Print the form and complete the demographic (top) portion of the form.
  • Ask your medical provider or nurse to complete and sign the Immunization History form (physician’s office address and phone number must be included). In lieu of completing the form, you may attach a copy of school immunization records, physician records or military records.
  • International Students: You must provide a certified English translation if immunization records are not in English. Please keep a copy of your immunization records in your personal files.
  • Upload your documents to MyMcKinley

  • If you experience difficulty with the upload process, you may fax your documents to (217) 244-1278, mail or hand deliver to:

            McKinley Health Center

            Urbana, IL 61801

  • Within 2 weeks of receipt, you will receive an email notice to check your compliance status using MyMcKinley.
  • If you are notified that additional vaccinations are required, please attempt to obtain those vaccinations prior to arrival on campus. 




2. Add your campus address/U.S. cell phone number to your student health portal and "Opt In" for text messaging

  • Log in at MyMcKinley
  • Click on your name in the top right corner of the screen
  • Click on Edit Your Profile
  • Edit your profile by adding your University of Illinois campus address and U.S. cell phone number
  • Check the box: "I would like to receive text messaging reminders" (to receive text appointment reminders)
  • Click on Save




3. Tuberculosis Screening (TB)/ Submitting QuantiFERON Gold Test Results [required for International Students ONLY]

TB screening is required for international students (including IEI). If you have had a QuantiFERON Gold (Q-Gold) test within 6 months of your enrollment date (for testing outside the U.S.) or within one year of your enrollment date (for testing within the U.S.), follow “Submitting Q-Gold results” instructions below: (See FAQ if you plan on obtaining the QuantiFERON Gold AND COVID-19 vaccination upon arrival to campus)

TB Screening Deadlines:

  • Submit your online TB Screening form by the deadline (Fall – July 1, Spring – December 1, Summer – April 1)


TB Screening form completion:

     -  Click on the Forms Icon

     -  Complete TB Screening form and submit by the deadline

    -  Once documents are reviewed by a nurse, you will receive a secure message on your MyMcKinley portal regarding TB testing requirements. 

Next steps:

  • If your secure message says no Q-Gold testing is required, no further action is necessary.
  • If your secure message says Q-Gold testing is required, obtain Q-Gold testing prior to arrival on campus. See steps below:


Submitting prior Q-Gold results:

  • To submit your Q-Gold results:
  1. Go to and log in with your net id and password
  2. On the MyMcKinley Home Page, click on the Upload Icon, select “Immunzation forms” from the dropdown to upload your documents

Obtaining required Q-Gold testing prior to campus arrival:

  • To obtain testing and submit Q-Gold results (if testing is available in your home country):
  1. Obtain the test within 6 months of your enrollment date (for testing outside the U.S.) or within one year of your enrollment date (for testing within the U.S.)
  2. Go to and log in with your net id and password, to submit results
  3. On the MyMcKinley Home Page, click on the Upload Icon and upload your documents

    5. Optional Disclosure of Private Mental Health Information​​ (not required)

    In compliance with the Student Optional Disclosure of Private Mental Health Act, the university has established procedures for students to have the opportunity to authorize, in writing, the disclosure of certain private mental health information to a designated person.
